Why Alcohol Wakes You at 2am in Perimenopause

You have a glass of wine to wind down, maybe two. You fall asleep easily. Then it is 2am and you are wide awake, mind switched on, staring at the ceiling. You are not anxious about anything in particular. You are just awake, and you cannot understand why.
Why alcohol makes you fall asleep faster and why that is the problem
Alcohol is a sedative, which is the whole reason a glass of wine feels like it helps: it slows your brain down and you drop off quickly. In the first part of the night it even pushes you into more deep, slow-wave sleep, but sedation is not the same as sleep.
Real, restorative sleep cycles through stages, including REM — the dreaming stage that does much of the work for memory and mood. Alcohol suppresses REM in the first half of the night.¹ You are sedated, but you are not getting the sleep you actually need.
What happens to your sleep in the second half of the night
Then the alcohol clears. Your body processes it at roughly one standard drink an hour. The REM it suppressed earlier comes flooding back in a rebound. Your sleep turns light and broken, and you surface in the small hours unable to settle.²
An Australian-led 2025 meta-analysis found that even a low dose of alcohol — around two standard drinks — disrupts REM sleep, and it gets worse with every extra drink. Faster sleep onset only appeared at a high dose, and the authors note that early sedation likely makes the later REM disruption worse.³

Alcohol, temperature and the hot flush connection
Here is the part that is specific to perimenopause. Alcohol widens your blood vessels and raises your core temperature. For many women, drinking in the evening makes hot flushes and night sweats more frequent or more intense — feeding the very disruption that is already breaking their sleep.
Why your tolerance has changed: the oestrogen piece
The most common thing women tell me: “Two glasses never used to touch me, and now they wreck my sleep.” That is real, and there is a reason.
Two things are at play. The first is body water. Alcohol spreads through the water in your body, so the more water you hold, the more diluted each drink becomes. Women hold less body water than men to begin with. As oestrogen falls through perimenopause, your body holds less water and carries a little more fat, so each drink creates a higher concentration in your blood.
The second is the enzyme that breaks alcohol down, called alcohol dehydrogenase. Women carry less of it in the stomach lining from the start, so more alcohol passes directly into the bloodstream. Oestrogen appears to influence how actively that enzyme works, so as your levels shift through perimenopause, your body may clear alcohol more slowly than it used to.
Put together, it means alcohol can hit harder and linger longer — including through the hours when sleep is most easily broken.⁴
What this means for you
There is one experiment worth doing. For two weeks, go alcohol-free and track your sleep — in particular the 2am to 4am window — noting whether the waking pattern changes. If the pattern shifts, you have your answer. If it does not, you have ruled something out and can look elsewhere. Either way you know more than you did.

3. Your tolerance really has changed. Slower metabolism and falling oestrogen mean the same two glasses hit harder and linger longer than they did a decade ago.⁴
This is not your body failing you. It is your body responding to less oestrogen — and asking for a different kind of consideration.
